Before you start

People establishing residence in Ontario. This is an Ontario guide, not the rule for other provinces or territories.

  • Check every part of the official eligibility criteria, including physical presence, primary residence and your qualifying category. The presence requirements describe ongoing conditions; do not interpret them as a new waiting period.
  • Prepare the registration form and three separate qualifying documents: citizenship/eligible status, Ontario residence and identity. The official list explains which residency documents can be printed or electronic and where originals are required.
  • Ask the centre what to bring for each child, spouse or dependant and whether they must attend. Do not assume one adult’s successful application enrolls the whole household.

Steps

  1. Check if you qualify
    Generally, you must make Ontario your primary residence, be present for 153 days in any 12-month period AND 153 of your first 183 days, and meet an eligible citizenship or immigration category. Exceptions exist. A work permit alone does not establish eligibility: Ontario lists employment and other conditions. Read the full official criteria or ask ServiceOntario.
  2. Collect three documents
    Bring one original document proving citizenship or OHIP-eligible immigration status, one document proving Ontario residency (original, printed, or digital as noted on the official list), and one original document proving your identity. Photocopies are not accepted.
  3. Complete the registration form
    Download the Registration for Ontario Health Insurance Coverage form from the official application page. Ask ServiceOntario which documents and attendance requirements apply to each family member; children and babies can follow different registration routes.
  4. Apply in person at a ServiceOntario centre
    OHIP is applied for in person. There is no longer a waiting period — if you are eligible, coverage is immediate. Every family member, including children, must have their own immigration document to register.

What to expect—and where people get stuck

Ontario has no OHIP waiting period for eligible applicants. Eligibility approval and delivery of a physical card are different things: ask what proof to use while waiting for the card.

If documents are missing, ask exactly which category is incomplete before returning. If ServiceOntario formally finds you ineligible, the official page explains the OHIP Eligibility Review Committee route and further appeal deadlines.

A health card is not a family-doctor appointment. Once covered, arrange ongoing care separately. Not every health service, prescription or dental treatment is paid for by OHIP.
